We all want to be healthy as individuals and as a community. This website is about measuring and engaging people in the health of their communities. We focus on leading indicators—the ones deemed by experts to make the difference in a community's health and quality of life.
This free data resource will help perform community health needs assessments, guide the development of community improvement plans, and much more.
The Indiana State Department of Health and the Indiana Hospital Association have together taken the lead in this endeavor, continuing to reach out and help communities and their hospitals obtain the metrics they need to monitor and evaluate health.
